Brachial plexopathy is a condition that can result in pain or motor impairment of the affected upper extremity. The plexopathy can be traumatic either due to direct penetrating injury or indirect injury secondary to stretching of the nerves. Birth brachial plexus palsy is a special category of traumatic plexus injury secondary to trauma during the delivery of the infants. The neoplasms of the brachial plexus can be primary or secondary to extension of the neoplasms from the thorax and neck. Pathologies like radiation plexopathy and neuropathy will need adequate clinical history to supplement the imaging diagnosis. Management of brachial plexopathy is quite complex due to the complex anatomy and the morbidity associated with it. Good communication of the findings by a radiologist in brachial plexopathy is crucial for appropriate management. Radiologists should be familiar with multimodality imaging anatomy of the brachial plexus, multimodality imaging protocol, and key radiological findings of various brachial plexus pathologies.
