The evolution of microsurgery training: a narrative review of historical foundations and modern innovations

Microsurgery, a cornerstone of reconstructive surgery, requires exceptional technical proficiency and precision, presenting unique challenges for surgical education. This narrative review traces the evolution of microsurgical training, from its historical roots in apprenticeship-based learning to the advent of structured curricula and cutting-edge innovations. Initially dependent on operating room (OR) exposure under expert guidance, training adapted to modern constraints, such as reduced resident work hours and heightened patient safety standards, through alternative methodologies. This review explores traditional approaches, including mentorship and wet labs, alongside contemporary advancements like simulation-based techniques and virtual reality (VR) platforms. Wet labs offer realistic hands-on practice with biological tissues, while VR and synthetic models provide scalable, repeatable environments that enhance psychomotor skills and dexterity. Synthesizing global literature, we highlight pivotal milestones in microsurgery education, assess the efficacy of modern tools, and address ongoing challenges, such as disparities in access and the need for standardization. These findings emphasize the importance of integrating historical lessons with innovative strategies to equip surgeons for the demands of microsurgical practice.
