<p>Diabetes negatively impacts the male reproductive system and <i>Heliotropium indicum</i> has been utilized in folklore to treat various human diseases, including female infertility owing to its wide spectrum of pharmacological activity. Thus, the present study investigated the effect of <i>H. indicum</i> leaf extract (AHI) on male reproductive parameters in diabetic rats. Thirty Wistar rats were randomly grouped into five; all groups except the control were fed a high-fat diet for 14&#xa0;days and 35&#xa0;mg/kg streptozotocin was administered. Group 3 was treated with 20&#xa0;mg/kg of acarbose (AC), while groups 4 and 5 received 50 and 100&#xa0;mg/kg AHI, respectively. The blood glucose level, antioxidant status, hormonal level, sperm quality, and morphology were assessed. Furthermore, the histological and histomorphometric parameters were examined. Findings in this study revealed a significant (<i>p</i> &lt; 0.05) decrease in antioxidant status, sperm quality, and hormonal level in the HFD/STZ group compared with the control. However, treatment with AHI significantly improved antioxidant status, sperm quality, and hormonal level concerning the HFD/STZ untreated group. Moreover, treatment with <i>H. indicum</i> restored the altered testicular histology and histomorphometric parameters. This study showed that <i>H. indicum</i> markedly improves male reproductive parameters in diabetic rats, and could therefore be a promising alternative therapy in combating male reproductive disorders associated with diabetes.</p> Graphical Abstract <p></p>
