Background <p>Rhinosinusitis is a common inflammatory disorder of the nasal cavity and paranasal sinuses, classified as acute or chronic according to symptom duration. Owing to the anatomical and physiological relationship between the sinonasal tract and the middle ear, rhinosinusitis may influence auditory function. However, the extent and characteristics of hearing impairment in this population remain unclear.</p> Objective <p>To systematically review and synthesize the available evidence regarding the prevalence, types, and associated factors of hearing loss in patients with acute and chronic rhinosinusitis.</p> Methods <p>This systematic review was conducted in accordance with PRISMA guidelines. A comprehensive search of MEDLINE, PubMed, Scopus, and Google Scholar was performed on February 14, 2026. Observational studies published in English that evaluated hearing impairment in patients with acute or chronic rhinosinusitis were included. Data extraction covered study characteristics, diagnostic methods, prevalence, and types of hearing loss. Methodological quality was assessed using the Joanna Briggs Institute checklist. Due to substantial heterogeneity, findings were synthesized narratively.</p> Results <p>Seven cross-sectional studies published between 2018 and 2025 were included. Reported prevalence of hearing loss ranged from approximately 3% to over 40%. Chronic rhinosinusitis demonstrated a stronger association with hearing impairment than acute rhinosinusitis. Conductive hearing loss was commonly observed in studies reporting middle ear involvement, whereas sensorineural hearing loss predominated in studies employing detailed audiometric and otoacoustic emission testing. Associated factors included older age, nasal polyps, facial pain, and disease severity.</p> Conclusion <p>Hearing impairment may represent a clinically relevant but variably expressed manifestation of chronic rhinosinusitis. Routine auditory evaluation should be considered in selected patients, particularly those with severe or longstanding disease.</p>
