Background <p>Olfactory disorder (OD) is a common but often underdiagnosed symptom in patients with chronic rhinosinusitis (CRS). As the sense of smell contributes significantly to life quality, appetite, safety, and emotional well-being, its impairment can profoundly impact daily functioning. Despite its importance, OD remains a neglected symptom in routine CRS assessment, with limited attention given to its actual burden globally. Various studies have suggested that the prevalence of OD in CRS is high, yet the estimates vary widely depending on the population, diagnostic methods, and definitions used. This inconsistency calls for a more comprehensive approach to understanding the scale of the problem.</p> Main body <p>This study aims to determine the worldwide occurrence of olfactory disorder amidst patients with chronic rhinosinusitis through a systematic review with meta-analysis. Using predefined eligibility criteria, after screening by title and abstract, 50 documents were identified. Then, these documents were subjected to full-text review. Twenty-nine studies were deemed eligible and added to the meta-analysis following the application of the final inclusion and exclusion criteria. The studies varied in geographical origin, patient characteristics, and methods of olfactory assessment.</p> <p>The analysis revealed that the overall global prevalence of olfactory disorder among CRS patients was 72.5%. This finding suggests that nearly three-quarters of individuals suffering from CRS also experience some form of smell dysfunction. More specifically, anosmia — a complete loss of the sense of smell — had a random-effects pooled prevalence of 40.2%, indicating a substantial burden of severe olfactory loss among this population.</p> Conclusion <p>The results highlight that olfactory disorder is not a minor symptom in CRS but rather a common and significant manifestation. The high prevalence rate underscores the need to incorporate routine smell assessment in evaluating CRS patients. Early detection of OD can lead to timely management, improved outcomes, and better quality of life. Clinicians should, therefore, consider smell testing a standard part of the diagnostic work-up in CRS.</p>
