Outcomes and complication rates of different materials used for nasal dorsal augmentation in septorhinoplasty: a systematic review and meta-analysis

Autologous materials and alloplastic implants are commonly used in dorsal augmentation of septorhinoplasty (SRP). So far, there is no satisfactory evidence to ascertain the safest, efficient, and superior aesthetic outcomes among different materials.
Objective This systematic review and meta-analysis (SR/MA) aim to compare the outcomes and complication rates of various materials used for nasal dorsal augmentation in SRP.
Methods A comprehensive search was conducted in PubMed, Embase, and Cochrane Library databases. Studies were included based on predefined criteria, and data were extracted on outcomes and complications. Statistical analyses were performed to compare the materials. Thirteen electronic databases were searched from inception through December 2018 and a manual search in October 2019.
Results A total of 280 eligible studies were included, of which 165 articles were included in MA. Alloplastic expanded polytetrafluoroethylene (e-PTFE) showed the highest graft scores across most outcomes with low complication rates. Silicone implants had a high success rate but slightly higher complications. Autologous materials such as bone and cartilage presented higher complication rates and donor site morbidity.
Conclusion e-PTFE appears to be the superior material for nasal dorsal augmentation due to its high patient satisfaction and low complication rates. Future studies should focus on long-term outcomes and racial differences in material performance.
