Background <p>Despite extensive efforts to promote road safety, road fatalities continue to rise. This study aimed to determine the road safety measure utilisation rate among road users involved in motorcycle and car accidents.</p> Methods <p>We prospectively analysed all cases between March 2019 and April 2020. Data collected included demographics, clinical parameters, imaging findings, management strategies, and outcomes. We summarised continuous and categorical variables in RStudio (4.3).</p> Results <p>164 participants were enrolled, with a male preponderance (73.8%). Participants ranged from 2 to 94&#xa0;years, with a mean age of 33.2 ± 18.4&#xa0;years, primarily within the third and fourth decades of life. Road safety measure utilisation was generally low at 20.1%, with the highest usage rate among drivers (30%), followed by pedestrians (17.2%) and riders (17.2%). Passenger utilisation was 7.1%, while pillion riders had the lowest rate at 4.8%. The observed mortality rate was 11.6%, with 75% (14/19) of deaths occurring in individuals who did not use safety measures. All motorcycle-related fatalities were associated with non-helmet use.</p> Conclusions <p>This study highlights suboptimal road safety measure utilisation among traumatic brain injury patients. Raising awareness, enforcement, and evidence-based strategies are essential to reduce the burden of traumatic brain injuries and improve road safety.</p>
