Background <p>Stunting remains a significant public health challenge, prompting ongoing exploration of effective mitigation strategies. Despite its importance, the genetic factors contributing to stunting are not well understood. This study aimed to investigate the determinants of stunting, considering the potential role of GHSR gene polymorphism in Indonesia.</p> Methods and results <p>The study used a cross-sectional design involving 110 children aged between 12 and 59 months. Stunting was defined as a height-for-age Z-score of less than − 2 standard deviations. Data were collected on parental height, education, per capita income, social health insurance, smoking behaviors, sanitation, household food security, birth anthropometry, children’s health history, access to healthcare facilities, early initiation of breastfeeding, and history of receiving iron tablet during pregnancy, as well as children’s nutrient intake. Blood samples were drawn for SNP analysis of the growth hormone secretagogue receptor (GHSR) gene (ΔQ36, G57G, P108L, L118L, R159R, C173R, D246A, and A277P&#xa0;polymorphisms) using RT-PCR and bi-directional sequencing. Binary logistic regression was employed to identify determinants of stunting. This study revealed that GHSR gene polymorphism (CT genotype&#xa0;for the variant G57G) was significant determinant of stunting in this population (AOR = 3.876; 95% CI = 1.085–13.859; <i>p</i> = 0.037).</p> Conclusion <p>This study underscores the importance of considering genetic factors in strategies aimed at reducing stunting.</p>
