Objective <p>The aim of this study is to investigate the clinical efficacy of combination therapy with ulinastatin and budesonide as an adjuvant treatment in patients with pelvic fractures complicated by pulmonary contusion.</p> Methods <p>A total of 136 individuals with pelvic fractures complicated by pulmonary contusion were enrolled from the Pelvic Ward of the Department of Trauma at Tianjin University Tianjin Hospital, Tianjin City, between July 2021 and July 2024. Participants were randomly assigned to a study group or a control group. The control group received budesonide suspension via aerosol inhalation (2&#xa0;mg per dose, every 6&#xa0;h). The study group received the same treatment along with intravenous administration of ulinastatin injection (1.0 × 10<sup>5</sup> U per dose, administered one to three times daily based on clinical condition). Both groups underwent treatment for two weeks. Clinical parameters assessed included the pulmonary contusion simple score, visual analogue scale (VAS), lung function and alveolar oxygenation function indices, lung function-related cytokine levels, immune function and oxidative stress indices, and the incidence of complications. The data were compared between the two groups.</p> Results <p>The study group demonstrated statistically significant improvements in pulmonary contusion simple score, VAS, lung function and alveolar oxygenation indices, lung function-related cytokine levels, immune function, and oxidative stress indices compared to the control group (<i>p</i> &lt; 0.05). However, no significant difference in overall complication rates was observed between the groups (<i>p</i> &gt; 0.05).</p> Conclusions <p>The combination of ulinastatin and budesonide demonstrated significant efficacy in improving lung function indices, modulating immune responses, and reducing inflammation and oxidative stress in individuals with pelvic fractures complicated by pulmonary contusion. The combination therapy exhibited a favorable safety profile, may serve as a potential treatment approach.</p>

Objective
