Assessment of operational preparedness of Ayushman arogya mandirs for comprehensive primary health care in Bhavnagar district of Western India

Ayushman Arogya Mandirs (AAMs) under the Ayushman Bharat programme are central to delivering comprehensive primary health care, aligned with the goals of ‘universal health coverage’ and ‘health for all’. Assessing their preparedness is essential to ensure effective service delivery.
ObjectivesTo assess the preparedness of Ayushman arogya mandirs in Bhavnagar district.
MethodsA cross-sectional assessment of 63 AAMs in Bhavnagar district was conducted from July 2021 to September 2022. Facilities were evaluated through physical inspection, record verification and staff interviews. Findings were summarized using descriptive statistics.
ResultsMost AAMs had dedicated buildings (85.7%), out-patient department rooms (100%) and waiting areas (82.5%); however, facility signages and staff information displays were poorly available (< 25%). Information, education and communication materials were largely displayed in waiting and out-patient areas. Human resources were largely adequate, with only four multipurpose female worker posts vacant. Over 80% of community health officers had completed core trainings. Essential registers like reproductive child health and referral register were missing in more than half of the facilities. Critical instruments such as rapid malaria and dengue test kits, autoclaves, and vision and dental screening tools were largely unavailable. Additionally, many AAMs did not achieve 75% of their monthly service targets.
ConclusionAyushman arogya mandirs in Bhavnagar district demonstrated satisfactory infrastructure, staffing, availability of essential medicines and training coverage of community health officers. However, operational gaps in record maintenance and logistics persisted. Addressing these gaps and strengthening infrastructure, logistics and training could further enhance the delivery of comprehensive primary health care services.
