<p>India faces a disproportionate share of the global antimicrobial resistance (AMR) burden, driven by high prevalence of infectious diseases and widespread misuse of antibiotics. Understanding the knowledge, attitudes, and practices (KAP) of future clinicians is essential for shaping effective stewardship strategies. We conducted a cross-sectional study in 2024 to assess KAP related to antimicrobial usage and AMR among medical interns and postgraduate residents in a government teaching hospital in Bengaluru, India. Data was collected through a structured, self-administered electronic questionnaire disseminated through institutional communication channels. The tool included items on sociodemographics and KAP (11 knowledge, 6 attitude, and 9 practice items). Bloom’s cut-off was used to categorize scores. Chi-square or Fisher’s exact tests were used to compare KAP scores across groups. Among 211 respondents (response rate: 70%), the mean age was 27 years (SD: ±2.5), and 71% were female. The mean scores for knowledge, attitude, and practice domains were 8/12, 18/27, and 15/21, respectively. Though 99% acknowledged the risks of indiscriminate antibiotic use, only 36% were familiar with the WHO AWaRe classification, and just over half (52%) understood the use of antibiograms. Non-clinical postgraduate residents had significantly better knowledge (<i>p</i> &lt; 0.001), while clinical residents demonstrated more appropriate attitudes (<i>p</i> = 0.024) and practices (<i>p</i> &lt; 0.001). Only 25% had attended any AMR-related training in the past year. Medical training institutions across India should integrate structured antimicrobial stewardship education into curricula and designate stewardship champions within clinical departments to promote and sustain evidence-based prescribing practices.</p>
