Efficacy and safety of scaling and root planing for non-alcoholic fatty liver disease

To evaluate the effects of scaling and root planing (SRP) on stage of hepatic steatosis measured by abdominal ultrasound (US) in patients affected by Metabolic Associated Fatty Liver Disease (MAFLD) and periodontitis.
MethodsIn this preliminary retrospective observational study, 205 adult patients affected by equivalent steatosis grade ≥ 1 and periodontitis grade ≥ 1 were included. Patients’ baseline characteristics included age, comorbidities and biochemical parameters (alanine aminotransferase, aspartate aminotransferase, gamma-glutamyl transpeptidase). Baseline periodontal grade was assessed using periodontal probing depth and grade of hepatic steatosis was assessed using abdominal US. Patients were subjected to periodontal treatment consisting in SRP, in addition to oral hygiene recommendations and the Mediterranean diet. After 6 months, the grade of hepatic steatosis was re-evaluated by an abdominal US. Statistical analysis with Pearson’s Chi-squared test was performed. This research received approval from the Ethics Committee.
ResultsNo significant correlation was observed between MAFLD severity and periodontitis stage. At 6 months follow-up, the degree of hepatic steatosis at abdominal US was significantly reduced in 89.8% of patients (p-value < 0.001), regardless of the grade of hepatic steatosis. No adverse events were reported.
ConclusionNon-surgical periodontal treatment consisting in SRP induced a significant short-term reduction of hepatic steatosis at abdominal US. Further research is mandatory to define SRP efficacy and safety in patients with MAFLD, and periodontitis.
