Toxocarosis, toxoplasmosis and ocular dirofilariasis diagnosed in a polish farmer from Wielkopolska Voivodeship – a case description

Zoonotic diseases caused by different parasites are recently more frequently diagnosed in humans. Contact with domestic animals, mainly dogs and cats increases a risk for the infection. Toxocarosis and dirofilariasis have worldwide distribution. Parasitic infections in humans are diagnosed incidentally because health care providers has a little knowledge about them. In this manuscript we described a case of a polish farmer with visual problems who was diagnosed multiple parasitic coinfection cased by Dirofilaria, Toxocara sp. and Toxoplasma gondii.
Materials and methodsA 63-year-old man admitted to the Infectious Diseases, Immunodeficiency and Hepatology Department because of visual disturbances. He was performed ophtalmological examination and nematode parasite was removed from the patient’s eye. Parasitic (microscopy) and molecular sections of the obtained nematode were done. To detect specific monoclonal antiparasitic (Toxocara – IgG and Toxoplasma gondii IgM and IgG) ELISA and Westren-blot were used. After the treatment the patient was followed up in the outpatient department of the Infectious Diseases Clinic.
ResultsThe patient was diagnosed coinfection with Dirofilaria repens, Toxocara sp. and Toxoplasma gondii infection. He was treated with surgical evacuation of the nematode and given oral albendazol and mebndazole.
ConclusionParasitic infections belong in many countries to the neglected illnesses and very often are diagnosed accidentally. The diseases spectrum varies dependly on the geographic location. Systemic parasitic coinfections, such as Toxocara spp. and Toxoplasma gondii can result in intraocular inflammation, which can be a major cause of blindness. Parasitic infections due to climate changes should also be taken more frequently under consideration in medical practice specially in patients with undifferentiated symptoms.
