Background <p>Malnutrition and atrial fibrillation (AF) are both important challenges in current health care. Malnutrition is associated with poor prognosis of various diseases such as AF, but the effect of malnutrition on the risk of AF is not yet clear.</p> Objective <p>The purpose of this study was to investigate the relationship between malnutrition and the risk of AF.</p> Methods <p>In this study, 13,702 cardiovascular inpatients in RED-CARPET and 10,505 community residents in ARIC were scored using the Controlled Nutritional Status (CONUT) score. We assessed the effect of malnutrition on the risk of AF. We used multivariate logistic regression to assess the association between malnutrition and AF in the RED-CARPET study, and used multivariate cox proportional hazard regression to verify the results in the ARIC study.</p> Results <p>In RED-CARPET, according to the CONUT score, 51.2% of the participants had malnutrition, including 43.8% of mild malnutrition, 7.0% of moderate malnutrition, and 0.4% of severe malnutrition. Compared with good nutritional status, malnutrition was associated with the prevalence of AF [the adjusted odds ratio (95%CI) of mild, moderate and severe malnutrition was 1.32 (1.12,1.55), 1.85 (1.45,2.36), 2.99 (1.50,5.99), respectively]. In ARIC, malnutrition was associated with the risk of incident AF, and the risk of AF increased with the aggravation of malnutrition [ the adjusted hazard ratio(95%CI) of mild and moderate malnutrition to AF were 1.33 (1.18, 1.51) and 2.04 (1.15, 3.62), respectively].</p> Conclusions <p>Malnutrition is associated with AF in both hospitalized and community populations, and the risk of AF increases with the degree of malnutrition in ARIC.</p> Trial registration <p>The RED-CARPET study was registered in the Chinese Clinical Trials Registry with the registration number ChiCTR2000039901.</p> <p>The ARIC study was registered in the clinicaltrials with the registration number NCT00005131. Background

Objective

Methods
