Abstract <p>Thalassemia is a prevalent genetic blood disorder in the Malakand Division, Khyber Pakhtunkhwa, Pakistan, primarily caused by consanguinity and lack of awareness. This cross-sectional study, conducted from October 2023 to June 2024, aimed to assess the prevalence, demographic determinants, and awareness levels of thalassemia in this region. Data were collected from 569 individuals through hospitals, thalassemia centers, and interviews. The study revealed that males, Rh-positive individuals, and the unemployed had the highest prevalence, with blood group B being the most affected. Additionally, the majority of patients were between the ages of 16 to 20. Awareness of the disease was low, with only 66% of respondents have knowledge about thalassemia, and 63% reported intermarriages in their families. The findings highlight the need for enhanced awareness programs and the introduction of premarital screening to mitigate the spread of thalassemia.</p>
