Measurement properties of the Wolf Motor Function Test for assessing upper limb function in Spanish adults with stroke

Upper limb impairment is one of the most common motor consequences after stroke, and valid assessment tools are essential for evaluating motor capacity. The Wolf Motor Function Test (WMFT) is widely used to measure upper limb function, yet evidence on its measurement properties in Spanish-speaking populations remains limited. This study aimed to evaluate the reliability, measurement error, construct validity, and discriminant validity of the Spanish version of the WMFT. A total of 139 individuals with stroke and 96 healthy participants were included. Internal consistency, test–retest reliability, and standard error of measurement were examined for performance time and functional ability scores. Construct validity was assessed through correlations with the Action Research Arm Test (ARAT), Box and Block Test (BBT), and Functional Independence Measure (FIM). Discriminant validity was explored by comparing stroke and healthy groups and by analysing discriminative accuracy. The WMFT exhibited excellent internal consistency for performance time and functional ability scores (α = 0.85, 0.94), along with good to excellent test–retest reliability (ICC = 0.79, 0.90). Convergent validity hypotheses were largely confirmed, and the test showed strong ability to differentiate between stroke participants and healthy controls, providing additional evidence of construct validity. However, Task 14 displayed lower correlations with the ARAT compared with the other WMFT tasks. These findings support the Spanish WMFT as a reliable and valid instrument for measuring upper limb motor capacity after stroke.
