<p>The transition from foundational anatomy education to applied surgical anatomy remains a persistent challenge in medical and surgical training. This study describes the development and preliminary evaluation of <i>Anatomy Unlocked</i>, a mobile-first, clinically oriented digital anatomy platform designed to integrate descriptive anatomical knowledge with functional, procedural, and surgical context. The platform combines expert-authored surgical notes with high-fidelity three-dimensional anatomical models, 360° manipulation, selective display and concealment functions, and model-derived anatomical diagrams. This architecture was designed to support spatial reasoning, active learning, and procedure-oriented navigation, while maintaining accessibility through a web-based, mobile-first interface. Preliminary assessment used a phased, mixed-methods approach across multinational learner cohorts. In the comparative educational efficacy study, students using the software achieved higher post-session assessment scores than those receiving traditional teaching alone in the combined cohort (13.37 ± 2.94 versus 11.95 ± 3.16; Mann–Whitney U = 2965.0, <i>p</i> = 0.0019; Cohen’s d = 0.47). Subgroup analysis showed significant gains for macroanatomy learning, whereas no significant difference was observed for functional anatomy. Usability evaluations among high school, university, and surgical course cohorts indicated favourable ease of use, perceived educational value, satisfaction, and support for understanding complex anatomy. These early findings suggest that a clinically oriented digital anatomy platform may improve short-term anatomical learning and learner engagement, particularly for spatially complex anatomical content. By linking three-dimensional visualisation with surgical landmarks, layered anatomical relationships, and procedure-focused content, <i>Anatomy Unlocked</i> provides a scalable framework for bridging preclinical anatomy education and applied surgical anatomy. Further longitudinal, multicentre studies using objective performance, competency-based, and clinical outcome measures are required to determine whether the observed improvements in usability, perceived understanding, and short-term assessment performance translate into durable anatomical reasoning, procedural competence, and clinical impact.</p>
