<p>To evaluate the quality, reliability, and penile prosthesis implantation (PPI) specific counseling content of top-ranked PPI videos surfaced by the default ranking algorithms of two Chinese short-video platforms. The top 100 default-ranked videos from TikTok and Bilibili were screened on 1 March 2026, yielding 179 eligible videos (92 TikTok and 87 Bilibili). Two independent reviewers assessed video characteristics, engagement, the Global Quality Score (GQS), modified DISCERN (mDISCERN), JAMA criteria, the Video Information and Quality Index (VIQI), and an exploratory six-domain PPI-specific content-coverage checklist. Platform comparisons used Benjamini–Hochberg false discovery rate correction. Spearman correlations and proportional-odds models examined associations of platform, duration, uploader source, and presentation format with quality scores. Bilibili videos were longer and had higher GQS (q = 0.017) and JAMA (q = 0.001) scores, whereas mDISCERN and VIQI did not differ between platforms. TikTok videos had higher absolute engagement, but interaction rate did not differ (q = 0.202). Duration-adjusted ordinal models retained higher odds of GQS and JAMA scores for Bilibili, whereas the fully adjusted estimates were attenuated after uploader source and presentation format were included together. Across the full sample, correlations between engagement metrics and quality scores were weak (Spearman ρ, − 0.134 to 0.216; all q &gt; 0.05). Complications were mentioned in 14.0% of videos and clearly explained in 5.6%; recovery was mentioned in 11.2% and clearly explained in 2.2%. Commercial bias and misinformation occurred in 6.7% and 7.3% of videos, respectively. Among top-ranked videos surfaced on the study date, general quality was moderate and PPI-specific counseling coverage was limited, particularly for complications and recovery. Platform differences were not explained by duration alone and were attenuated after uploader source and presentation format were considered. Popularity metrics should not be used as proxies for educational quality.</p>
