The effect of Roy adaptation model-based education on self-care and coping in patients with plaster casts: a single-blind randomized controlled trial

Patients with orthopedic conditions undergoing plaster treatment experience both physical limitations and psychosocial stressors. These challenges may hinder self-care practices and adaptive coping strategies. The Roy adaptation model, by focusing on promoting holistic adaptation, provides a valuable framework for nursing interventions aimed at enhancing patient outcomes. This study aimed to determine the effect of nursing education based on the Roy adaptation model on the self-care agency and coping attitudes of patients undergoing lower extremity cast treatment. A randomized, single-blinded, pre-test–post-test controlled experimental design was used. The study was conducted with 65 patients who were randomly assigned to either the experimental (n = 31) or control (n = 34) group. The experimental group received structured education and counseling based on the Roy adaptation model. Data were collected using the Exercise of Self-Care Agency Scale and the Coping Orientation to Problems Experienced Inventory before and after the intervention. Statistical analyses included independent and paired t-tests, and Cohen’s d was used to calculate effect sizes. Post-intervention, the experimental group had significantly higher mean scores on the Exercise of Self-Care Agency Scale compared to the control group (p = 0.008, d = 0.727). Similarly, significant improvements were observed in several subscales of the Coping Orientation to Problems Experienced Inventory, particularly in the use of instrumental and emotional social support, positive reinterpretation, and suppression of competing activities (p < 0.05). The intervention was associated with moderate to high effect sizes in key outcomes. The findings suggest that nursing education based on the Roy adaptation model may support self-care agency and some adaptive coping attitudes in patients receiving lower extremity cast treatment. Integrating theory-driven education into orthopedic nursing care may be a useful approach to support patients’ adaptation during the plaster treatment period.
