<p>Mpox (formerly known as monkeypox) has re-emerged as a global health threat. However, little is known about public awareness and responses in urban African populations. The Republic of Congo has experienced sporadic Mpox outbreaks over several decades, with Brazzaville, the capital, identified as the epicenter of the 2025 outbreak. Despite this, no study has assessed the knowledge, attitudes, and practices (KAP) of the population regarding Mpox. This study aimed to evaluate the KAP of Congolese residents toward Mpox. A cross-sectional, community-based survey was conducted in March 2025 in Brazzaville, targeting health districts with confirmed Mpox cases. A three-stage cluster sampling method was used to select individuals aged 15 and older from diverse social backgrounds. Data were collected through a standardized KAP questionnaire and analyzed using multivariable logistic regression to identify factors associated with knowledge, perceptions, and practices. A total of 567 participants were enrolled (46% male, 54% female). Overall, 89.6% (95% CI 86.8–91.8) demonstrated poor knowledge of Mpox, while 10.4% (95% CI 8.2–13.2) achieved a good knowledge score. Only 7.4% (95% CI 5.5–9.9) correctly identified a virus as the cause, and 38.0% (95% CI 34.1–42.0) had previously heard of the disease. Moderate to low-risk perception was observed in 61.0% (95% CI 57.1–65.1), and 67.0% (95% CI 63.0–70.8) of participants exhibited poor preventive practices. Better knowledge was associated with secondary education (aOR = 2.48; 95% CI 1.38–4.45), middle income (aOR = 0.13; 95% CI 0.05–0.35), and female gender (aOR = 0.35; 95% CI 0.18–0.64). Government employees and residents of Djiri and Talangaï districts reported better practices. Only 28% expressed strong trust in a hypothetical Mpox vaccine, and 19% believed that most community members would accept vaccination. Despite the ongoing outbreak, knowledge, perception, and preventive behaviors related to Mpox remain alarmingly low in Brazzaville. There is an urgent need for expanded public awareness campaigns and enhanced communication strategies to build trust and strengthen outbreak preparedness and response in the Republic of Congo.</p>
