Relationship between transversus abdominis thickness and inter-recti distance across body positions and motor tasks in nulliparous women

In this observational, single-session, cross-sectional study, a simultaneous ultrasonographic assessment of the inter-recti distance (IRD) and transversus abdominis (TrA) thickness was performed in 26 nulliparous women aged 18–26 years (21.2 ± 2.06 years) across different body positions (standing and supine), under resting conditions and during a motor task performed without voluntary TrA activation. The main objectives of the study were to determine whether morphological changes in the transversus abdominis reflect proportional changes in the distance between the rectus abdominis muscles across four conditions: supine and standing positions at rest and during a motor task. No correlation was found between the relative (percentage) changes in IRD and TrA thickness. However, resting TrA thickness varied by position and was significantly greater in standing than in supine (p = 0.008; 95% CI [0.096, 0.59]; d = 0.49). TrA thickness increased significantly during the motor task in standing position compared to resting values (p = 0.003; 95% CI [-0.57, 0.13]; d = 0.34). When it came to the motor task in supine position compared to resting values the changes in TrA thickness were not significant (p = 0.049; padj = 0.29, 95%CI [-0.32, 0]; d = 0.22). The IRD tended to decrease during the motor task in both standing and supine positions compared to rest (p > 0.05). TrA thickness increased in standing and during the motor task, but these changes were not accompanied by proportional changes in IRD. These findings offer baseline data for future studies involving symptomatic women with diastasis recti abdominis (DRA).
