<p>Stroke survivors in China face substantial barriers to secondary prevention, including limited access to education and risk-factor self-management support. This study aimed to evaluate the short-term usability and feasibility of <i>Mai 20</i>, a user-centered mobile health (mHealth) app designed to support secondary stroke prevention and risk management. A prospective mixed-methods feasibility study was conducted at Minhang Hospital in Shanghai, China. Outcomes included stroke knowledge, usability, and willingness to use. Of the 37 enrolled participants, 30 completed the 7-day follow-up. Mean stroke knowledge increased from 8.2 (± 5.3) at baseline to 25.8 (± 4.7) after 7 days (<i>P</i>&lt;.001). The mean System Usability Scale score was 58.8 (± 11.0), indicating moderate usability; and the mean willingness-to-use score was 81.5 (± 8.5); 60% (18/30) of participants reported high or very high willingness to use the app. Qualitative feedback highlighted perceived benefits in education, medication reminders, clinician–patient communication, and health monitoring, while also identifying the need for simpler navigation and a reduced data-entry burden. Mai 20 demonstrated short-term feasibility and was associated with substantial short-term gains in stroke knowledge over a 7-day period; these findings support further usability refinement and larger multicenter trials with longer follow-up to evaluate sustained engagement, behaviour change, and clinical outcomes.</p>
