Holistic oncologic management improves quality of life in driver gene negative advanced NSCLC a latent growth curve modeling study
摘要
Associations between holistic oncologic management (HOM) and the trajectory of quality of life (QoL) in patients with non-small cell lung cancer (NSCLC) have not been evaluated, especially the driver-gene-negative NSCLC patients, whose QoL was proved to be poorer. This randomized controlled trial is performed from October 23, 2019, to July 25, 2023. Patients diagnosed with driver-gene-negative NSCLC were enrolled in Chongqing University Cancer Hospital and were classified into 2 groups: the control arm (arm C) and the treatment arm (arm T). The holistic integrative oncologic therapy was strictly implemented in the arm T, focusing on four basic elements: QoL, depression, pain and nutrition. Patients were comprehensively intervened and assessed in 24 consecutive weeks (i.e. four weeks/cycle, six cycles in total). Six-point QoL trajectories were derived from latent growth curve modeling (LGCM) analyses. The QoL was evaluated by the Functional Assessment of Cancer Therapy–Lung (FACT-L). Participants in two groups were 81 and 77, respectively. The HOM was shown to significantly enhance the QoL, alleviate cancer-related pain, and improve nutritional status and mood symptoms in driver-gene-negative NSCLC patients. Notably, younger patients have experienced more significant improvements. HOM improves the QoL in patients with driver-gene-negative advanced NSCLC, especially in patients with younger age. Trial Registration The research was retrospectively registered on 11/11/2024, in the Chinese Clinical Trial Registry (http://www.chictr.org.cn/ChiCTR2400092132).
