<p>Sustained eosinophilia can indicate a serious health issue and may elevate the risk of thrombosis. This study aimed to explore potential risk factors of thrombosis in eosinophilia. This case-control study enrolled patients with an absolute eosinophil count of ≥ 500 cells/µL at the First Medical Center of the Chinese People’s Liberation Army General Hospital between January 2010 and January 2022. Multivariable logistic regression analysis was conducted to identify risk factors for thrombosis in eosinophilia. A total of 88 patients were included, with 40 (45.5%) experiencing thrombosis. The median age of patients with thrombosis was 42.5 years (range: 30.3 to 58.8), and 34 (85.0%) were male. In contrast, the median age of patients without thrombosis was 51.0 years (range: 30.3 to 66.0), with 34 (70.8%) being male. Peripheral neuropathy was significantly more prevalent in patients with thrombosis compared to those without (35.0% vs. 12.5%, <i>p</i> = 0.012) and was identified as an independent risk factor for thrombosis (odds ratio: 3.39; 95% confidence interval: 1.02–11.47; <i>p</i> = 0.045). Peripheral neuropathy may be a risk factor for thrombosis in patients with eosinophilia. This study highlights the importance of identifying patients at risk for thrombosis, providing valuable insights for their management.</p>
