A cross-sectional study on the epidemiology and risk factors for falls among the elderly in Chongqing based on the China elderly fall surveillance initiative

The aging population is associated with an increasing burden of falls worldwide. However, the epidemiological characteristics of falls in Chongqing, China remain poorly understood. To assess the prevalence and factors associated with falls in an elderly population aged 60 and older in Chongqing municipality, China. This cross-sectional study was conducted from April 2022 to December 2022 based on China Elderly Fall Surveillance Initiative (CEFSI). Multi-stage stratified random sampling of individuals aged 60 and older, representative of both urban and rural areas of Chongqing was enrolled. The weighted prevalence of falls and associated risk factors, stratified by age, sex, and urban versus rural residence, was determined by electronic questionnaire. A total of 3072 participants was included in this study (1534 women [49.93%]; mean [SD] age: 73.01 [7.42] years), all of whom underwent a free medical examination and completed the questionnaire. The prevalence of falls among individuals aged 60 and older was 20.28% (95% CI 19.24–22.11%), with significantly higher rates in women (25.29%, 95% CI 23.74–28.13%) versus men (15.28%, 95% CI 13.06–16.61%) 95% CI 14.12–17.78%, and in rural (24.09%, 95% CI 22.62–26.94%) versus urban (16.47%, 95% CI 14.12–17.78%) areas, and peaking at 25.78% among the 80-year-old age group. According to the multivariable logistic regression analysis, self-rated health status is poor (OR 2.24, 95% CI 1.65–3.04), afraid of falling (OR 1.78, 95% CI 1.43–2.20), even if not injured, one should not tell family or doctor after a fall (OR 1.68, 95% CI 1.32–2.14), and female gender (OR 1.60, 95% CI 1.32–1.95) were the top four risk factors. A high prevalence of falls among adults aged 60 and older in Chongqing, particularly among women and in rural areas. A comprehensive strategy encompassing clinical, public health, and societal interventions is essential to mitigate this significant public health issue and improve the well-being of the aging population.
