<p>Diabetes mellitus patients frequently develop hypertension, which reduces quality of life and increases mortality and morbidity. Even though hypertension is a common co-morbidity, determinants of hypertension among diabetes mellitus patients are scarce. The purpose of this study is to identify the determinants of hypertension among adult diabetic patients on treatment follow-up. A hospital-based unmatched case-control study was conducted from January 1, 2023 to February 15, 2023. Using a sampling frame of patients registered in each hospital in the chronic care register logbook, a simple random computer-generated sampling technique was used to select cases and controls using a medical recode number. Diabetes was confirmed by checking the physician’s diagnosis on their chart/medical record. Data were entered using EpiData version 4.2 and exported to SPSS version 25 for coding, cleaning, and analysis. Multivariable logistic regression analysis was used to identify determinate variables at a p-value &lt; 0.05 with a 95% confidence interval. This study included 370 patients (124 cases and 246 controls), with a 99.5% response rate. The participant’s mean (SD) age was 49.65<i>+</i>14.63 years. Sixty-eight (54.8%) cases and 131 (53.3%) controls were males, whereas 85 (65.5%) cases and 137 (55.7%) controls resided in urban. Age 40–60 years [AOR: 4.30 (95% CI: 1.84, 10.07)] and &gt; 60 years [AOR: 3.91 (95% CI: 1.65, 9.27)], urban residence [AOR: 1.72 (95% CI: 1.02, 2.75)], alcohol use [AOR: 2.10 (95% CI: 1.25, 3.53)], sleeping habits [AOR: 2.01 (95% CI: 1.20, 3.34)], overweight [AOR: 2.22 (95% CI: 1.33, 3.71)], and poor glycemic control [AOR: 2.57 (95% CI: 1.46, 4.53)] were identified as determinates of hypertension in patients with Diabetes. Improving and strengthening awareness about behavioral and lifestyle modifications are required to reduce the development of hypertension among diabetes mellitus patients.</p>
