<p>This article introduces the concept of transition of care from child to adult services and describes the development of a transitional pathway by the Lancashire Teaching Hospitals National Health Service Trust paediatric and adult special care dental teams. Case reports of patients managed within the pathway are presented to illustrate implementation of the pathway and the various considerations that formed part of the individual care plans. It is hoped that this work will support other dental services to develop arrangements that improve the quality of transitional care for patients.</p>
