Abstract <p>Necrotizing enterocolitis (NEC) reveals substantial heterogeneity in presentation and underlying mechanisms. Conventional diagnostic tools, especially radiographic criteria and Bell staging, fail to distinguish between biologically distinct forms of intestinal injury, leading to imprecise management and research practices. The continued search for universal biomarkers is hindered by a lack of NEC mechanistic clarity. This commentary discusses abandoning the monolithic “NEC” label in favor of a data-driven, cluster-based taxonomy that integrates clinical trajectories with multiomic analyses.
