<p>This meta-analysis aimed to evaluate the effectiveness and safety of the minimally effective dose and optimal concentration of oral glucose for pain relief during painful procedures. Systematic review and meta-analysis were conducted following PRISMA guidelines. Protocol was registered with PROSPERO (CRD42021236217). PubMed, Cochrane, Embase, Web of Science, and Scopus were searched until September 2024. Continuous data were analyzed using inverse-variance fixed effects analysis. Risk of bias and GRADE assessment were performed using RoB 2.0 and GRADEpro tools. Sixteen RCTs with 500 preterm infants were included. Meta-analysis revealed that 25% glucose significantly reduced pain compared to breast milk during heel lance at 30 s. No significant differences in heart rate (HR) or oxygen saturation (SpO₂) were observed between glucose and breast milk. However, 25% glucose significantly reduced HR and improved SpO₂ levels compared to placebo. Oral glucose is an effective, safe pain management strategy in neonatal care.</p><p></p>
