Objective <p>Severe asthma can limit social and work life and negatively impact emotional well-being. This study aimed to evaluate loneliness among patients with severe asthma.</p> Methods <p>This cross-sectional study was conducted between December 2023 and June 2024 in an allergy clinic. Patients diagnosed with severe asthma and treated with systemic steroids or biologics were included. Data were collected through face-to-face interviews using a patient information form, the Asthma Control Test (ACT), and the UCLA Loneliness Scale.</p> Results <p>A total of 84 patients (mean age: 48.8 ± 9.6&#xa0;years; 69.0% female) were enrolled. The median UCLA score was 41.0 (range: 26–72), with 29.8% showing mild, 42.9% moderate, and 27.4% severe loneliness. Severe loneliness was significantly higher among patients using systemic corticosteroids (86.7%) compared to non-users (14.5%) (<i>p</i> &lt; 0.001), and among those with uncontrolled asthma (75.0%) compared to those with controlled asthma (16.2%) (<i>p</i> &lt; 0.001). Biologic therapy was associated with lower UCLA scores (<i>p</i> &lt; 0.001). A moderate negative correlation was found between ACT and UCLA scores (r = – 0.628, <i>p</i> &lt; 0.001).</p> Conclusions <p>Severe loneliness in asthma patients is linked to poor asthma control and systemic steroid use. Biologic therapy may help reduce loneliness.</p>
