<p>Cardiovascular diseases (CVDs) are leading cause of death globally. The need for alternative therapeutic interventions that are accessible, cost-effective, and have least adverse effects has become necessary. Herb-infused honey is a fusion of bioactive phytochemicals of both honey as well as herbs that promise as an adjunct remedy in managing CVDs. Herbal medicines also have potential to produced considering the least toxic side effects and serious drug-drug interactions that necessitate scientific evaluation. This review summarizes the ethnopharmacological properties of medicinal herbs that are used in herb-infused honey formulations, including <i>Terminalia arjuna</i>, <i>Crataegus oxyacantha</i> L., <i>Piper longum</i>, <i>Allium sativum</i>, <i>Trigonella foenum-graecum</i>, <i>Commiphora wightii</i>, <i>Curcuma longa</i> L., <i>Ocimum tenuiflorum</i> L., <i>Zingiber officinale</i>, <i>Foeniculum vulgare</i>. Experimental evidence suggests that it may exert cardioprotective effects through attenuated oxidative stress, reduces platelet aggregation, alters inflammatory pathways, decreased LDL oxidation, angiogenesis promotion, and vasodilation. The preclinical and nascent clinical studies have demonstrated that it influence multiple molecular pathways that include TNF-α, IL-6, NF-κB inhibition, SOD, catalase, GSH level modulation, and HMG-CoA reductase &amp; CYP7A1-based regulation of lipid and cholesterol metabolism, thereby reducing the overall cardiovascular burden. The lack of standardization of bioactive compounds, variability in the composition of honey and herbs used, optimization of dosage, and the need for large-scale randomized controlled trials are main limitations in clinical implementation. The pharmacokinetics, bioavailability, and possible interactions of herb-infused honey with conventional cardiovascular drugs are essential to understand before its clinical use. This translational review underscores the potential of herb-infused honey as a functional cardioprotective agent that bridges traditional wisdom with evidence-based medicine.</p> Graphical Abstract <p></p>
