<p>The infection caused by <i>Echinococcus granulosus</i> presents a significant global public health concern, impacting both animal and human populations. This paper examines the prevalence, risk factors, and consequences of this infection in goat and sheep populations across East Africa. Through an extensive literature review, twenty relevant studies from Tanzania, Kenya, Ethiopia, South Sudan, Somalia and Uganda from PubMed and Google Scholar were identified. Inclusion criteria focused on <i>E. granulosus</i> infection in small ruminants in East Africa (2013–2023), emphasizing prevalence and risk factors. Exclusions included studies outside East Africa or lacking peer-reviewed data. Data on prevalence rates, geographical location, animal species, and sampled organ types were extracted and analyzed. The highest prevalence among small ruminants was reported in South Sudan, with rates of 65.2% in goats and 67.8% in sheep. Factors influencing disease prevalence included geographical location, animal species, and sampled organ type. The impacts of the disease ranged from heightened zoonotic risks to economic losses for livestock owners. Notably, Kenya observed significant financial implications. This review underscores the critical necessity for One Health control strategies and targeted interventions to mitigate the impact of <i>E. granulosus</i> on both animal and human health in East Africa. Addressing these issues is essential for reducing zoonotic risks and economic burdens associated with the disease.</p>
