Background <p>The assessment of bone mineral density (BMD) by dual-energy X-ray absorptiometry (DXA) of the spine and hip bone is the recommended practice for the detection of central osteoporosis. However, the use of distal radial BMD may become essential where hip or spine assessments are not feasible due to prior surgeries or fractures. Moreover, ethnic variation in BMD necessitates a regional assessment of diagnostic accuracy of distal radial BMD in identifying central osteoporosis, which we aimed to explore among Bangladeshi adults.</p> Methods <p>This cross-sectional study enrolled 275 participants aged &gt; 50&#xa0;years with clinical suspicion of osteoporosis from a university hospital and a private clinic in Bangladesh. BMD was measured at the lumbar spine, femoral neck, and distal radius using DXA. Receiver operating characteristic (ROC) curves were constructed to evaluate the diagnostic performance of radial BMD in detecting osteoporosis at the spine and hip sites. Sensitivity, specificity, and predictive values were calculated.</p> Results <p>The mean age of the participants was 65.19 ± 7.64&#xa0;years, and 34.55% and 11.27% had osteoporosis of the spine and hip, respectively. Distal radius BMD demonstrated the highest diagnostic accuracy (AUC = 0.857). For hip osteoporosis, total radial BMD had 77.42% sensitivity, 84.84% specificity, 39.34% positive predictive value (PPV), and 96.73% negative predictive value (NPV). For spine osteoporosis, total radial BMD showed 85.26% sensitivity, 67.22% specificity, 57.86% PPV, and 89.63% NPV.</p> Conclusion <p>Distal radial BMD measurements exhibited lower sensitivity but higher specificity for diagnosis hip osteoporosis and the opposite for spine osteoporosis in Bangladeshi adults with a high negative predictive value.</p>
