<p>Poorly cohesive gastric carcinoma is one of the rarest and most aggressive forms of adenocarcinoma. It is histologically characterized by the presence of more than 50% signet ring cells filled with mucin. A 40-year-old female was admitted to the hospital with abdominal discomfort and symptoms of gastroesophageal reflux disease for a duration of 2&#xa0;months. Upper gastrointestinal endoscopy revealed a flat gastric polyp with a central ulcer at the junction between the body and the antrum, classified as a type 0-IIa + III lesion according to the Paris classification. Biopsy results confirmed poorly cohesive gastric carcinoma. The patient underwent laparoscopic near-total gastrectomy with Roux-en-Y reconstruction and lymphadenectomy. Histopathological examination confirmed a diagnosis of diffuse gastric carcinoma, signet ring cell type, with negative resection margins and no lymph node involvement. The patient recovered well postoperatively and was discharged in stable condition. This case highlights the importance of early detection and timely surgical intervention in achieving a curative outcome for poorly cohesive gastric carcinoma. <i>Clinical trial number:</i> Not applicable.</p>
