Fatigue is a common symptom in primary care practices that can have a variety of causes, which remain partly undefined. It can occur as part of mental disorders such as depression or anxiety disorders, but also as a symptom of many somatic diseases such as anaemia, thyroid dysfunction, diabetes mellitus and sleep apnoea. More rarely, causes such as malignancies or myalgic encephalopathy/chronic fatigue syndrome (ME/CFS) are the underlying cause. A biopsychosocial approach to diagnosis, which includes a thorough anamnesis, physical examination and focused basic laboratory tests, is essential. Therapy depends on the individual causes and can range from activating measures and behavioural therapy to the specific treatment of an underlying disease. In the case of ME/CFS in particular, an appropriate approach such as pacing is crucial in order to avoid overload.
