Background <p>Ethiopia's dominance in distance running is well-documented, yet the systemic factors leading to high rates of athlete burnout and injury remain unexamined. This study addresses the "Ethiopian Paradox": the coexistence of unparalleled success and significant athlete attrition within the same high-performance system. Guided by the Holistic Athlete Development Model, this research investigated the interplay of training, psychological, and socio-cultural factors.</p> Methods <p>A retrospective, mixed-methods cohort study was conducted with 60 retired elite Ethiopian endurance runners. Two cohorts were compared: a "Durable &amp; Thriving" group (n = 30; careers ≥ 10&#xa0;years) and a "Burnout &amp; Injury" group (n = 30; careers ≤ 7&#xa0;years). Quantitative data from historical training logs (used to calculate a Training Load Volatility Index) and psychological surveys (ACSI-28; MPS) were analyzed. Qualitative data from semi-structured interviews underwent thematic analysis. Findings were integrated to develop a predictive "Durability Profile."</p> Results <p>Quantitative analysis revealed the Durable cohort exhibited significantly lower training load volatility (<i>p</i> &lt; .01, d = 0.85), higher scores on coping with adversity (<i>p</i> &lt; .05, d = 0.65), and lower scores on maladaptive perfectionism (<i>p</i> &lt; .01, d = 0.90). Qualitative thematic analysis identified three core themes differentiating the cohorts: "Guided Autonomy" in coaching, "Integrated Athletic Identity," and "The Buffer of Community." Integration of these findings produced a three-pillar "Durability Profile" model.</p> Conclusion <p>Career longevity in Ethiopian elite runners is not solely a product of physiological talent. Instead, it is fostered by a synergistic combination of intelligent training-load management, specific psychological resilience traits, and robust socio-cultural support structures. The "Durability Profile" provides an evidence-based framework for coaches to cultivate sustainable, long-term success in athlete development.</p>
