The case of a&#xa0;34-year-old woman with psoriasis and inflammatory back pain illustrates the relevance of early imaging and interdisciplinary management. Beyond nonsteroidal anti-inflammatory drugs (NSAIDs) and biologics, nonpharmacological measures such as exercise and patient education are essential. Persistent fatigue and nociplastic pain remain challenging and highlight the need for multimodal strategies to improve quality of life, even when inflammation is well controlled.</p>
