Right ventricular function plays a central role in perioperative and critical care management of severely ill patients, despite receiving little clinical attention. Even though it is prognostically relevant, right ventricular dysfunction is often detected late. Conventional hemodynamic parameters such as central venous pressure or pulmonary artery pressure provide only indirect information about right ventricular function. In contrast, analysis of the right ventricular pressure waveform allows direct assessment of right ventricular hemodynamics. In addition to pressure amplitude, the morphology of the waveform in particular provides important information about contractility, afterload, and diastolic function. This article reviews the physiology of the right ventricle, describes the characteristics of the right ventricular pressure waveform, and highlights its clinical relevance in perioperative and critical care settings.
