<p>The indication and patient selection criteria are central to successful outcomes in cartilage surgery. An existing joint degeneration, such as osteoarthritis, remains the primary contraindication. However, no precise parameters currently exist to provide a&#xa0;definitive prognosis for therapeutic success. Therefore, patient suitability must be assessed based on multiple factors. This article outlines the authors’ clinical approach, which—owing to a&#xa0;lack of comprehensive scientific evidence in many areas—deliberately draws on extensive clinical experience. The aim is to encourage a&#xa0;critical evaluation of indications and to provide suggestions for optimizing patient selection in the future.</p>
