Aims <p>This study analyzes the clinical presentations, symptoms, and outcomes of foreign body aspiration (FBA) in pediatric patients at a tertiary care hospital in South India over six years, aiming to improve understanding and management of this critical condition.</p> Materials and methods <p>A cross-sectional study was conducted through a retrospective review of 272 children (under 18&#xa0;years) admitted from January 2018 to December 2023, suspected of FBA and undergoing rigid bronchoscopy. Data collected included demographics, symptoms, radiological findings, timing of bronchoscopy, and the need for oxygen support.</p> Results <p>Of 69,697 children admitted with respiratory complaints, 271 (2.66%) underwent bronchoscopy for suspected FBA, with foreign bodies confirmed in 164 cases (60.51%). The majority of cases (56.1%) were infants and toddlers (0–2&#xa0;years). Common symptoms included cough (62%), fever (26%), and choking. Organic materials, such as groundnuts (52.43%) and areca nuts (31.09%), were the most frequent foreign body types. Radiographic sensitivity for diagnosing FBA was low at 57%, with a negative predictive value of 39%.</p> Conclusion <p>Foreign body aspiration remains a significant pediatric emergency, particularly in young children. This study underscores the importance of high suspicion and timely bronchoscopy for effective management. Enhanced parental education on prevention and the availability of trained pediatric surgeons in district hospitals are essential to mitigate risks associated with FBA.</p>
