Background <p>This study aimed to evaluate the serum levels of IgG-4 and anti-TPO antibodies in patients with autoimmune hypothyroidism and to determine their correlation with thyroid function parameters.</p> Methods <p>This cross-sectional study was performed on newly diagnosed autoimmune hypothyroid patients from March 2022 to March 2024 at the Endocrinology Department of Ghaem Hospital (Mashhad, Iran). The serum levels of IgG-4, TSH, Anti-TPO, and Free T4 were measured and based on the IgG-4 levels, patients were divided into two groups of autoimmune hypothyroidism associated with IgG-4 (&gt; 135&#xa0;mg/dl) and the unrelated group (IgG-4 ≤ 135&#xa0;mg/dl). Data analysis was performed using SPSS version 22 software.</p> Results <p>Participants were adults aged 25–60 years (median 36; IQR 31–43). Of 44 patients, 79.55% female, and 20.45% male, were included in this study, and 7 (15.9%) were IgG-4 positive and 84.1% were IgG-4 negative. Although the TSH and Anti-TPO antibody levels were higher in the IgG-4 positive group, none of the variables of age, gender, Free T4, and Anti-TPO level were significantly different between the two groups. There was only a significant correlation between TSH and IgG-4.</p> Conclusion <p>These preliminary findings suggest that a subset of autoimmune hypothyroid patients exhibits elevated IgG-4 levels. While our data do not support the routine measurement of IgG-4 for clinical management, evaluating this marker serves as an exploratory starting point. External validation in larger populations is required to determine its true utility in understanding disease progression.</p>
