<p>As the most prevalent cancer among women worldwide, breast cancer is frequently managed with chemotherapy, which remains central across neoadjuvant, adjuvant, and metastatic indications. While these regimens have substantially improved disease-free and overall survival, they are frequently associated with toxicities that compromise quality of life (QoL). This narrative review synthesizes current evidence on the multidimensional impact of chemotherapy in patients with breast cancer, encompassing physical, psychological, social, and economic domains. Common adverse effects include fatigue, neuropathy, gastrointestinal distress, hematologic toxicities, and cardiotoxicity, each of which can persist beyond treatment and impair daily functioning. Psychological sequelae such as depression, anxiety, cognitive impairment, and body image disturbance further diminish well-being. Additionally, chemotherapy disrupts sexual health, family roles, work productivity, and imposes significant financial burden, particularly in resource-limited settings. QoL measurement tools, including EORTC QLQ-C30, QLQ-BR23, FACT-B, and SF-36, provide valuable patient-reported outcomes that complement clinical metrics. Supportive interventions such as exercise, psychosocial counseling, pharmacological management, and nutritional strategies have demonstrated benefits in mitigating chemotherapy-related burden. However, heterogeneity in study design and patient subgroups limits generalizability. As breast cancer survivorship expands, integrating QoL considerations into treatment planning is essential. Future research should emphasize longitudinal, patient-centered assessments and interventions to optimize both survival and well-being.</p>
