Background <p>Passive smoking is a significant public health issue linked to an increased risk of cardiovascular diseases (CVDs). Despite extensive research in smoking-related health risks, the evolution and global impact of studies on passive smoking and CVDs remain inadequately characterized. This bibliometric analysis aims to evaluate publication trends in this domain over the past two decades.</p> Methods <p>A bibliometric analysis was performed using Scopus and PubMed databases, covering the period from January 2000 to December 2024. Studies focused on passive smoking and cardiovascular diseases are included. Data were extracted in RIS format and analyzed using the Bibliometrix package in R studio. Metrics such as annual publication outputs, citation patterns, influential journals were used for this study.</p> Results <p>The analysis identified a total of 3,322 documents sourced from 1,189 journals, with an annual publication growth rate that peaked in 2020. High-income countries (HICs), especially the United States(1004 articles), emerged as dominant contributor for a substantial share of the global research output. <i>PLOS ONE</i> (98 articles)and <i>International Journal of Environmental Research and Public Health</i> (70 articles) emerged as top journals. The University of California (333 articles) emerged as the leading institution. Institutional collaborations were concentrated in high-income countries.</p> Conclusion <p>This bibliometric study elucidates the dynamic research landscape on passive smoking and CVD, underlining both the progress made and critical gaps that remain. Study highlighted the disparities in global research equity. Strengthening global collaborations and expanding research into emerging tobacco products are essential to mitigate the health burden of passive smoking and advance evidence-based policy making.</p>
