Neuropathic Pain Following Spinal Cord Injury: A 32 Years Bibliometric Analysis of Research Trends, Key Contributors, and Future Directions

Neuropathic pain (NP) following spinal cord injury (SCI) is a prevalent and debilitating condition that profoundly affects quality of life, employment, and sleep. Despite its high prevalence, management of SCI-related NP remains challenging due to the limited efficacy of current pharmacological treatments.
ObjectivesThis study aimed to assess research trends, key contributors, and emerging directions in the scientific literature on SCI-related NP from 1992 to 2024, providing a comprehensive overview of the field’s evolution and thematic focus.
MethodsA bibliometric analysis was conducted using literature data retrieved from the Web of Science Core Collection (WoSCC) and Scopus databases. Data processing and analysis were performed in RStudio (Bibliometrix package) and VOSviewer to examine publication trends, citation performance, and co-occurrence networks. A total of 1189 English-language articles published across 416 journals were analyzed to identify influential authors, institutions, and research hotspots.
ResultsThe findings reveal a steady increase in publication output over the past three decades, with the USA emerging as the leading contributor. The most productive institution was the University of Miami, and PAIN ranked as the most influential journal. Keyword co-occurrence and cluster analyses identified five major thematic areas: neuroinflammation, central pain mechanisms, rehabilitation strategies, neuroplasticity, and neuromodulation-based therapies.
ConclusionsThe bibliometric mapping highlights a maturing yet evolving research landscape in SCI-related NP. Although substantial advances have been made in understanding underlying mechanisms and developing therapeutic strategies, significant gaps remain—particularly regarding standardized diagnostic tools and translational efficacy. The study underscores the need for interdisciplinary collaboration and the integration of emerging technologies, such as neuromodulation and molecular-targeted interventions, to improve patient outcomes.
