Dyslipidemia and Its Association with Accelerated Atherosclerosis in Rheumatoid Arthritis Patients: A Cross-Sectional Study

Rheumatoid arthritis (RA) is a chronic inflammatory disorder mainly affecting synovial joints, leading to disability and increased early mortality. Systemic inflammation in RA is associated with higher cardiovascular morbidity and mortality. Dyslipidemia, marked by abnormal lipoprotein metabolism, is common in RA patients and contributes to faster atherosclerosis, raising cardiovascular risk. This study aimed to assess the prevalence of dyslipidemia among RA patients receiving care at a tertiary hospital in Karachi.
MethodsA cross-sectional study was carried out at Abbasi Shaheed Hospital over six months, enrolling 196 RA patients who met the diagnostic criteria after providing informed consent. Quantitative data are shown as mean ± standard deviation; categorical variables are presented as frequencies and percentages. Associations were examined using chi-square tests, with p ≤ 0.05 indicating statistical significance.
ResultsAmong 196 RA patients (mean age 54.14 ± 4.49 years), 44.9% had dyslipidemia. Females constituted 74.5% of the cohort, with dyslipidemia significantly more prevalent in females (50%) than males (30%) when adjusted for group sizes (p = 0.01). The highest prevalence was in the 41–50 age group (45.4%). Common comorbidities included type II diabetes (37.8%) and hypertension (46.9%). Stratification by socioeconomic status showed higher dyslipidemia prevalence in lower-income groups, but no significant associations were found with disease duration, diabetes, hypertension, smoking, or occupation. Family history of dyslipidemia did not significantly correlate with dyslipidemia status.
ConclusionDyslipidemia affects nearly half of RA patients at a tertiary care center, highlighting its importance as a key cardiovascular risk factor in this group. Early identification and treatment of dyslipidemia in RA are crucial to reducing cardiovascular morbidity. Larger studies with multivariate analyses and more diverse patients attending the hospital are needed to better understand risk factors and improve care.
