Incidence and Risk of Tachyarrhythmia in Cancer: A Systematic Review and Meta-analysis of Observational Studies

The risk of developing cardiovascular diseases such as heart failure, ischemia, or arrhythmia is higher in cancer patients. This systematic review and meta-analysis aimed to estimate the incidence and risk of Tachyarrhythmia in cancer patients.
MethodsWe searched PubMed, Scopus, Cochrane Library, Web of Sciences, Embase, and Scholar databases based on mesh terms to find studies that investigated the incidence and risk of Tachyarrhythmia in cancer patients with no time limit up to April 2025. The study was conducted based on the PRISMA checklist. Cochran's Q and I2 tests were used to evaluate heterogeneity between studies. Sensitivity analysis and meta-regression were used to control heterogeneity—Egger's test examined publication bias.
ResultsFinally, eighteen studies, including 291,464 cancer patients, were included in the study. The pooled incidence of Tachyarrhythmia in cancer patients was 15% (95% CI: 8, 22%; I2: 99.9%). The pooled incidence of Tachyarrhythmia and ventricular tachycardia (VT) was 17% and 10%, respectively. The incidence of Tachyarrhythmia varied by cancer type. A pooled estimate of 12 studies showed that cancer or related chemotherapy was significantly associated with an increased risk of tachyarrhythmia (HR: 1.79, 95% CI (1.49, 2.08), I2 = 94.7%). Subgroup analysis showed that chemotherapy increased the risk of tachycardia and VT by 37% and 47%, respectively.
ConclusionsThe overall incidence of Tachyarrhythmia was high in cancer patients. Cancer and chemotherapy may be associated with an increased risk of tachyarrhythmia. The risk varied depending on the type of tumor, and the risk of Tachyarrhythmia was higher in patients with lung cancer.
