Purpose <p>To report a successful amniotic membrane transplantation (AMT) for painful decompensated cornea using a trephination-assisted inlay technique and to highlight its long-term efficacy and advantages over conventional methods.</p> Materials and Methods <p>Case report.</p> Result <p>A 39-year-old gentleman presented with chronic left eye pain for months. His left eye was blind since 10&#xa0;years old due to treated traumatic retinal detachment. Examination revealed left corneal scarring and band keratopathy with pseudo-pterygium formation. There was no fundus view. He was offered AMT after frequent failure of bandage contact lens use and refusal of evisceration. He underwent pseudo-pterygium excision and chelation of band keratopathy at the same setting. The AM was tucked into the pre-trephined cornea groove via inlay transplantation and secured with nylon 10/0. He recovered well and remained pain free post-operatively.</p> Conclusion <p>AMT is an effective alternative for treating painful decompensated cornea, as it can greatly enhance a patient’s quality of life while also offering cosmetic benefits.</p>
