Role of Preoperative and Postoperative Intravitreal Anti-VEGF Agents in Managing Diabetic Macular Edema and Diabetic Retinopathy

Diabetic retinopathy (DR) is the most prevalent microvascular complication of diabetes and a primary cause of blindness globally. Diabetic macular edema (DME), a major cause of vision impairment in DR, often progresses rapidly and requires timely and effective treatment, along with good glycemic control, to preserve visual function. Standard interventions such as laser photocoagulation and vitrectomy are effective in preventing vision loss by limiting DME progression, but postoperative bleeding remains a significant concern. The administration of intravitreal anti-vascular endothelial growth factor (anti-VEGF) agents before vitrectomy has been shown to reduce intraoperative and postoperative bleeding. This manuscript reviews the rationale, timing, and effectiveness of preoperative anti-VEGF injections in the surgical treatment of proliferative diabetic retinopathy (PDR) and DME. It also discusses the advantages of incorporating both pre- and postoperative anti-VEGF therapy in the management of DME associated with DR.
