Enhanced Level of Cardiac Biomarkers (Troponin-I, Atrial Natriuretic Peptide) and Angiotensin-II in COVID-19 Hospitalized Acute Coronary Syndrome Patients

The aim of this study was to compare the serum levels of Troponin-I (Trop-I), Atrial Natriuretic Peptide (ANP), and Angiotensin II (Ang-II) in hospitalized COVID-19 ACS patients and controls. Eighty-eight cardiovascular patients were divided into two groups based on infectivity with COVID-19, i.e., SARS-CoV-2 infected (n = 44) and non-infected patients (n = 44). Serum levels of Trop-I, ANP, and Ang-II were determined through ELISA, and the cardiovascular questionnaire was used to record the attitudes and experiences of respondents towards cardiovascular health. The levels of cardiac biomarkers and Ang-II were elevated in all ACS patients. However, the derangement was more pronounced in COVID-19 positive patients (Trop-I (p < 0.001), ANP (p = 0.002), and Ang-II (p < 0.001)). Cardiac markers were correlated with diabetes and clinical markers like loss of taste and loss of smell. In multivariate regression analysis, only Trop-I (Beta = 0.393, p ≤ 0.001) and Ang-II (Beta = 0.365, p ≤ 0.001) were independently related to COVID-19 positivity. The rise in cardiac biomarkers (Trop-I, ANP) and Ang-II in ACS COVID-19 patients compared to non-COVID-19 could be attributed to the increased myocardial stress due to hypoxia or virus-related myocardial injury causing a more severe disease course.
