<p>“Mylohyoid boutonnière” is a frequently seen defect in the mid-part of the mylohyoid muscle. Infrequently, when large, this defect may lead to herniation of sublingual glands, which may present clinically as a neck mass. We present the case of a 50-year-old lady with slowly increasing neck swelling, with repeated inconclusive results from palpation-guided fine needle aspiration cytology, where radiological imaging provided definite evidence of a mylohyoid defect with sublingual gland herniation. Knowledge of this pseudo lesion is important, as early imaging in such cases can help avoid unnecessary patient anxiety and invasive sampling.</p>
